Queensland Horse InterZone Declaration
For movements into and out of the green zone.
One copy of the Declaration will be generated after submitting the form.
Please print the Declaration twice and sign both copies.
One is to be retained at the place of departure and the other must accompany the horses to the destination.
The forms must be retained for two years.
Where a return journey is intended please supply a return date on the form, and the same declaration can be used for your forward and return journeys.
This form may be completed by any person, however the name of the legal owner of the animal must be given in section 1.
Where more than 1 legal owner is involved a separate form will need to be completed.
The form must be signed by the occupier at the place of departure or the legal owner or authorised agent.
Where an authorised agent makes the declaration and signs the forms they must have made sufficient enquiry about the health status of the animals to be able to make a declaration that is not false or misleading.
